Element 7 x 2 Sparkles - duplicate, mirror and flip. 

Put one on top and one on the bottom

Element 122 Wordart - resize and rotate to desired

 position

Element 14 Glass - resize and place in the bottom right

Element 13 Box - resize and place bottom right, right 

of the glass
